This a joint venture between the Hospice and South Warwickshire University Foundation Trust, providing a supportive space for parents/carers/guardians of young people affected by a life-limiting condition currently under transition of their health and social care from children to adult. We recognise that it is difficult for parents/carers/guardians to come together and share their experiences, find social connection and support, at a time of challenging changes and being able to have time together to talk and also access professional support through our Transitional Care Nurses and Therapists can be vital.
