Hospice at Home

Whether you are the patient, a carer or family member you will have time to talk about what is important to you. We will listen with sensitivity and compassion and help ease any anxieties that you or your family may have.

Your confidentiality is important to us. We will always seek your consent before liaising with other health care professionals or those who are important to you.

Once we have received your referral, we will:

• Arrange to visit you in your home or preferred place of care
• Carry out a full nursing assessment
• Give support and advice to plan your ongoing care
• Organise a visiting schedule
• Help you fulfil your physical and psychological needs
• Provide our emergency contact details
• Provide exceptional care in conjunction with our community healthcare partners

Our Hospice at Home service offers:

• Experienced nursing care for patients approaching the end of their lives, who wish to remain at home, surrounded by those who are important to them
• Specialised care and advice to prevent hospital admission
• Emotional and psychological support for patients and their families and carers
• Expert knowledge in assessment and management of physical symptoms, including administration of medications
• Liaison with the appropriate professionals to ensure that patients’ medications are effective for their symptom management
• A plan of care designed around their needs and wishes
• Advice and support provided from 8.30am to 10pm seven days a week, 365 days a year
• Access to other beneficial services at The Shakespeare Hospice
• Advice and signposting to other community services
• Pre & Post bereavement support
