Our Hospice in Shottery is the ‘hub’ for our day services. Our Hospice is a friendly, caring environment which provides support for those affected by life-limiting illness to access a wide range of services and therapies.
Our Hospice at Home service is for patients with life-limiting illnesses who have chosen to receive palliative and end of life care from the comfort of their own homes.
CAFSS is a free service available to children and young people (aged 5-18), who have had a sudden or expected bereavement or are facing the loss of someone close to them who has a life-limiting illness.
Our Adult Counselling service is available to patients and their carers or loved ones who are affected by a life-limiting condition and individuals who have been bereaved.
The Transitional Care Service supports young people between the ages of 16 and 24 who have been diagnosed with a life-limiting illness. The service provides a coordinated and seamless transition for young people, who are moving from paediatric care into adult health and social care services.
The best way to refer somebody to The Shakespeare Hospice is by completing our online referral form. The form can be completed by anyone, whether you’re a patient, family member / friend, or a healthcare professional.
