Being diagnosed with a life-limiting condition can be a lonely and isolating experience. Our Companion at Home service offers vital emotional and social support to individuals diagnosed with life-limiting conditions who may be experiencing loneliness or isolation.

This befriending program connects patients with trained volunteers who provide companionship, a friendly listening ear, and the opportunity to form meaningful friendships. The service also includes short respite visits to allow caregivers time for personal errands and support for attending appointments or walks, subject to conditions.

Companion at Home is designed to bring comfort, connection, and reassurance to those in need—all in the familiarity of their own home.

Am I eligible?

Our Companion At Home service is available to anyone over the age of 18 patient who has been diagnosed with a life-limiting condition and may be experiencing loneliness and isolation

Individuals can either be referred by a health care professional or can self-refer via our referral page.
